A Medicare-certified home-health agency needed a site that earns trust fast — in the family's own language. I built a clear six-service care catalog in four languages (EN/ARM/RU/ES), with two separate funnels: one for physician referrals and one for patient care requests.

The problem

Families choosing home care are making a high-stakes decision, often quickly, and they need instant trust and clarity — in a language they actually read. A single-language site would leave much of that audience guessing about what the agency does and who it serves.

The agency also had a second audience the usual patient-inquiry form ignores: physicians who send referrals. Referrals and care requests are different conversations, and funneling both through one generic contact form served neither well.

The approach

I organized the offering into a clear six-service care catalog, so a family or a referring physician can see exactly what's covered at a glance. Every page exists in four languages — English, Armenian, Russian, and Spanish — so the site meets each visitor where they are.

Then I split intake into two separate funnels: a physician-referral path built for clinicians and a care-request path built for families. Each collects the right details for its audience, so the agency knows immediately how to respond.

The result

The agency now presents itself with clarity and trust across four language communities, with a care catalog anyone can scan in seconds and two clean intake paths that route physicians and families to the right place.
